Anyone know why the digital download was delayed until 10/16?  What reason could they have for releasing the boxed version for PC, but not the digital?

I finally got the controller to work after I uninstalled my Nostromo game-pad software.

I ran into the same problem - there's something about Dark Souls that prevents the 360 controller from working if there's a Nostromo hooked up.  From has admitted they're not that knowledgable about PC development, which is why this seriously AWESOME game has such a half-baked PC port (ie, why we've had to have a 3rd party patch to unlock decent PC resolutions, etc).  That said, I hope the PC version does well enough that they consider investing in hiring some PC experts for the next game.
